Establishing Care

All patients are required to complete intake paperwork prior to their first visit. If you are unable to complete the intake paperwork online through the mconsent link sent to you, we ask that you arrive 20 minutes early to ensure we have adequate time for your appointment. Please keep in mind that if you show up at the time of your appointment start time and do not have your paperwork completed, you will need to be rescheduled.

Late Policy

Any patient arriving 7 or more minutes late must reschedule to allow us to remain on time for other patients. It is best if you arrange to arrive 5-10 minutes early for your check-in. While we do appreciate calls to let us know you are running late, you may still need to reschedule.

Medication Refill Requests

Our policy states that patients should allow 72 business hours to fulfill medication refill requests thus we ask that you plan accordingly. Unfortunately, we have had patients call multiple times a day to ask about the status of the refill and this will not expedite the refill. If you call on a Friday, Saturday, or Sunday, you should not expect those requests to be fulfilled immediately, as stated above.

​Clinic Hours/Holiday Closures

Our hours of service are 8 am to 5pm Monday through Thursday, and we close for lunch from 12-1pm. We periodically are able to see some patients on a Friday if arranged in advance. We are closed on traditional holidays: New Year’s Day, Thanksgiving, Christmas, Memorial Day, and Labor Day. Always check the website for updates.

DOT Physicals

We do perform DOT physicals however they are billed separately from
annual physicals and your insurance.

​NO SHOW FEE

If there is a no show for your appointment you will be charged a fee of 30 dollars. This is billed directly to the patient and not insurance. Please try to give us 24 hours advance notice if you cannot make your appointment or if you need to reschedule so we can offer the time slot to another patient. Please remember we are a small independently owned business and thus no shows affect us significantly.

Insurance

Before your visit, please contact your insurance to confirm that we are in-network for your specific plan. This will help ensure your services are covered.
